1. Definitions. "Activities" means skateboarding, coaching, drills and related physical activities. "Media" means photos and video recordings. "Skatemate" means the business trading as Skatemate (ABN 924977265333) and its coaches, staff and representatives.
2. Acknowledgement of risk. I understand that skateboarding involves inherent risk of injury, including but not limited to bruises, cuts, sprains, fractures, and in rare cases more serious injury. I voluntarily accept these risks on behalf of myself and/or the participant/s named above.
3. Fitness to participate. I confirm that each participant is in good general health and fit to take part. I will notify Skatemate in writing of any medical condition, injury or allergy relevant to the activities.
4. Safety and instructions. Participants will follow all reasonable instructions from Skatemate coaches, wear the safety gear supplied or recommended (helmet at minimum), and behave in a way that keeps themselves and others safe.
5. Release. To the extent permitted by law, I release Skatemate from any claim for loss or injury arising from the ordinary risks of the activities, except where caused by Skatemate's gross negligence or wilful misconduct. Nothing in this waiver excludes rights under the Australian Consumer Law.
6. Media consent. I consent to Skatemate taking photos or short videos during sessions and using them on Skatemate's website and social media for the purpose of promoting the business. If you don't want media used, tick "no media" when contacting us and let your coach know at the session.
7. Privacy. The information on this form is collected so Skatemate can run sessions safely and contact you. It is stored securely and not shared with third parties except where required by law.
8. Signature. By typing your full name below and submitting, you are signing this waiver electronically and confirming that the information provided is true and that you have authority to sign for each participant listed.
